Changes to AHPC SF2 Form (Undertaking by Supervisor)

Please be informed that we have recently made changes to our SF2 form (Undertaking by Supervisor).

Please be informed that we have recently made changes to our SF2 form (Undertaking by Supervisor). 

The main change in this version would be in the declaration section.

We have included a clause indicating that the supervisor and supervisee should not be personally related to each other. In addition, the supervisee should not be employed by any person whom he/she is personally related to. 

The new version of the SF2 form is now available for download under the "Downloads and Forms" section.
